Indications
Burnsil Cream is specifically formulated for:
- Prophylactic use against bacterial colonization and infections in burn wounds.
- Antibacterial management of contaminated or infection-prone wounds beyond burns.
Pharmacology
Burnsil Cream contains Silver Sulfadiazine, an effective antibacterial agent with a unique mechanism. It disrupts bacterial cell membranes, causing structural damage and reducing bacterial viability. The silver component binds to bacterial DNA, inhibiting cell proliferation, while the sulfadiazine provides additional bacteriostatic action. This ensures effective treatment while minimizing impact on epithelial cell regeneration.
Dosage & Administration
- Cleanse burn wounds thoroughly before application.
- Apply a layer of Silver Sulfadiazine Cream (about 1/16 inches or 1.5 mm thick) once or twice daily.
- Ensure the cream covers the burn area at all times; reapply if removed due to patient activity.
- Use dressings if needed and reapply after hydrotherapy.
- Continue application until healing is satisfactory or the burn area is ready for grafting.
Interaction
- Enzymatic Debriding Agents: May be inactivated by Burnsil; avoid concurrent use.
- Oral Hypoglycemic Agents and Phenytoin: Monitor blood levels as their action may be potentiated in patients with large burns.
- Cimetidine: May increase the incidence of leukopenia in patients with extensive burns.
Contraindications
- Hypersensitivity to Silver Sulfadiazine or any other ingredients.
- Not recommended for pregnant women at or near term, premature infants, or newborns under 2 months of age.
Side Effects
Potential side effects include transient leukopenia, skin necrosis, erythema multiforme, skin discoloration, burning sensation, rashes, and interstitial nephritis.
Pregnancy & Lactation
- Pregnancy Category B: Use only if clearly needed, especially in women nearing term.
- Safety during lactation is unclear; consider the importance of the drug versus the need for breastfeeding.
Precautions & Warnings
- Monitor hepatic and renal functions to prevent drug accumulation.
- Be cautious when using topical proteolytic enzymes, as silver may inactivate them.
- Regularly check serum sulfa concentrations and renal function in patients with extensive burns.
