Indications
Ciprofbrate is prescribed as an additional therapy alongside diet, exercise, and weight management for:
- Severe Hypertriglyceridemia: Effective for treating high triglyceride levels, whether accompanied by low HDL cholesterol or not.
- Mixed Hyperlipidemia: Suitable when statins are not an option due to contraindications or intolerance.
Pharmacology
Ciprofbrate works by reducing LDL and VLDL cholesterol levels, thereby lowering triglycerides and associated cholesterol. It also boosts HDL cholesterol levels. This medication is beneficial in managing hyperlipidemia linked to elevated LDL and VLDL levels and may help reduce coronary heart disease events.
Dosage & Administration
- Adults: The standard dose is 100 mg once daily. Do not exceed this dose.
- Renal Impairment: For moderate renal issues (creatinine clearance 30-80 ml/min/1.73 m²), use every other day. Avoid use in severe renal impairment (creatinine clearance <30 ml/min/1.73 m²).
- Hepatic Impairment: Use cautiously in liver dysfunction. Discontinue if AST or ALT levels exceed three times the normal limit or if cholestatic liver injury occurs.
- Elderly: Follow adult dosing but with additional precautions for those over 70 years.
- Pediatric Use: Not recommended due to insufficient safety and efficacy data.
Interactions
- Fibrates & HMG CoA Reductase Inhibitors: Risk of myopathy and rhabdomyolysis may increase with concurrent use.
- Oral Anticoagulants: Adjust dosage carefully and monitor INR levels.
Contraindications
- Allergies: Hypersensitivity to Ciprofbrate or its components.
- Hepatic & Renal Impairment: Severe liver or kidney dysfunction.
- Pregnancy & Lactation: Contraindicated during pregnancy and breastfeeding.
Side Effects
- Common: Headache, dizziness, somnolence, nausea, vomiting, diarrhea, dyspepsia, abdominal pain, rash, alopecia, myalgia, and fatigue.
Pregnancy & Lactation
- Pregnancy: Ciprofbrate is contraindicated as there is no human data and high doses have shown toxicity in animal studies.
- Lactation: Contraindicated due to lack of data on safety in breastfeeding.
Precautions & Warnings
- Galactose Intolerance: Not suitable for those with galactose intolerance or glucose-galactose malabsorption.
- Muscle Issues: Report any unexplained muscle pain or weakness immediately.
- Liver Function: Regular hepatic function tests are recommended; discontinue if liver enzyme levels significantly increase.
