Indications
Nervalin is prescribed for:
- Neuropathic pain associated with diabetic peripheral neuropathy (DPN)
- Postherpetic neuralgia (PHN)
- As an adjunctive therapy for partial-onset seizures in patients 1 month of age and older

- Neuropathic pain due to spinal cord injury

Pharmacology
Pregabalin, a structural derivative of the inhibitory neurotransmitter gamma-aminobutyric acid (GABA), does not bind directly to GABAA, GABAB, or benzodiazepine receptors. It binds with high affinity to the alpha2-delta site in central nervous system tissues, which is thought to contribute to its anti-nociceptive and antiseizure effects, although the exact mechanism is not fully understood.
Dosage
- Neuropathic pain (DPN) in adults: Start with 50 mg three times daily (150 mg/day), potentially increasing to 300 mg/day within a week. Maximum dose: 100 mg three times daily (300 mg/day). For Pregabalin CR, begin with 165 mg once daily, increasing to 330 mg once daily within a week. Maximum dose: 330 mg once daily.
- Postherpetic neuralgia (PHN) in adults: Start with 75 to 150 mg twice daily or 50 to 100 mg three times daily (150 to 300 mg/day), potentially increasing to 600 mg/day. For Pregabalin CR, begin with 165 mg once daily, increasing to 330 mg once daily within a week. Maximum dose: 660 mg once daily.
- Fibromyalgia in adults: Start with 75 mg twice daily (150 mg/day), potentially increasing to 300 mg/day within a week. Further increase to 450 mg/day if needed.
- Neuropathic pain due to spinal cord injury in adults: Start with 75 mg twice daily (150 mg/day), potentially increasing to 600 mg/day within a week.
- Conversion to Pregabalin CR: Switch from Pregabalin capsules to Pregabalin CR capsule after an evening meal. Follow specific dosage conversion recommendations.
Administration
Pregabalin is taken orally, with or without food. Pregabalin CR tablets should be taken after an evening meal, swallowed whole without splitting, crushing, or chewing. If a dose is missed, it should be taken with the next meal or before bedtime.

Contraindications
Nervalin is contraindicated in patients with known hypersensitivity to Pregabalin or any of its components.

Pregnancy & Lactation
There are no adequate studies in pregnant women. Pregabalin is found in small amounts in breast milk, and due to potential risks, breastfeeding is not recommended during treatment.
Precautions & Warnings
- Angioedema and hypersensitivity reactions require immediate discontinuation.
- Increased risk of suicidal thoughts or behavior.
- Respiratory depression may occur, especially with concomitant CNS depressants.
