Description

Indications

Anleptic CR serves as an effective treatment for various conditions, including:

  • Partial and secondary generalized tonic-clonic seizures
  • Primary generalized tonic-clonic seizures
  • Trigeminal neuralgia
  • Prophylaxis of bipolar disorder

Pharmacology

Carbamazepine, the active ingredient in Anleptic CR, works by suppressing activity in the nucleus ventralis of the thalamus. It achieves this by reducing the synaptic propagation of excitatory impulses or limiting the influx of Na ions across cell membranes. Additionally, it stimulates the release of antidiuretic hormone (ADH), enhancing its action in water reabsorption.

Dosage & Administration

For epilepsy treatment, dosages vary based on age:

  • Adults and children over 12: Starting with 200 mg b.i.d. for tablets or controlled release tablets, or 1 teaspoon q.i.d. for suspension (400 mg/day), increasing weekly as needed.
  • Children 6-12: Starting with 100 mg b.i.d. for tablets or controlled release tablets, or 1/2 teaspoon q.i.d. for suspension (200 mg/day), with similar incremental increases.
  • Children under 6: Initial dosage ranges from 10-20 mg/kg/day, adjusted weekly for optimal response.

Combination Therapy

Anleptic CR can be used alone or with other anticonvulsants. When combined, gradual dosage adjustments are recommended.

Trigeminal Neuralgia Treatment

Starting with 100 mg b.i.d. for tablets or controlled release tablets, or 1/2 teaspoon q.i.d. for suspension (200 mg/day), dosage can be increased by up to 200 mg/day as needed for pain relief, with a maximum daily dose of 1200 mg.

Interaction

Anleptic CR may interact with hepatic enzyme inducers like Phenytoin, potentially requiring dosage adjustments.

Contraindications

Avoid Anleptic CR if allergic to its ingredients or in conditions such as atrioventricular block, bone marrow depression, porphyrias, allergy to tricyclic antidepressants, or recent MAOI usage.

Side Effects

Common side effects include dizziness, drowsiness, gastrointestinal discomfort, and skin reactions. Monitoring for severe reactions, especially in the hematopoietic, skin, and cardiovascular systems, is advised.
