Indications
Migranil is recommended for the preventive treatment of vascular headaches, including classical migraine, common migraine, and cluster headache. It is not effective for relieving ongoing migraine attacks.
Description
Migranil is a tricyclic compound that competitively blocks serotonin receptors. It possesses antihistamine, antibradykinin, and mild anticholinergic properties, along with appetite-stimulating effects.
Pharmacology
Pizotifen, the active ingredient in Migranil, inhibits serotonin and histamine actions in the periphery, reducing membrane permeability of cranial vessels and transudation of plasmakinin. It alters pain thresholds in migraines and blocks 5-HT receptors, thereby lessening cranial vasoconstriction and platelet function. It may also inhibit bradykinin actions and serotonin reuptake by blood platelets, leading to decreased distension of extracranial arteries.
Dosage & Administration
For adults, the usual dose is 1.5 mg daily, taken as a single dose at bedtime or in three divided doses. Dosage can be adjusted up to a maximum of 4.5 mg daily. For children over 2 years, up to 1.5 mg daily is recommended, usually divided, though up to 1 mg can be taken as a single dose at night.
Interaction
Migranil may enhance the central effects of sedatives, hypnotics, antihistamines (including some cold medications), and alcohol. It also counteracts the hypotensive effects of adrenergic neuron blockers.
Contraindications
Pizotifen is contraindicated in individuals with known hypersensitivity to any of its components.
Side Effects
Common side effects include increased appetite, weight gain, and drowsiness.
Pregnancy & Lactation
Limited information is available on the use of Pizotifen during pregnancy, but no adverse effects have been reported.
